REQUIREMENTS FOR AIR CARRIERS FROM EUROPEAN ECONOMIC AREA PLUS SWITZERLAND
1. The documents required to be submitted together with the application for single commercial flight are:

Air Operating Certificate with Operations Specifications

Insurance Certificate

Operating Licence
Further document can be required on demand especially if there are specific safety or security concerns.
The application for a permission containing the required data and documents, should be submitted before the planned commencement of carriage services within period not shorter than 3 working days.
2. The air carrier intending to perform series of unscheduled commercial flights comprising of at least ten flights is required to submit the same documents as set forth in point 1.
The application for a permission containing the required data and documents, should be submitted before the planned commencement of carriage services within period not shorter than 14 working days.
3. The air carrier intending to perform scheduled flights is required to submit also:

Timetable before the beginning of every IATA season

Tariffs applicable to air transport services, for information of the authority

The application for a permission containing the required data and documents, should be submitted before the planned commencement of carriage services within period not shorter than 30 working days.
4. The air carrier intending to perform air transport within the territory the Republic of Poland must be aware of the following:
According to the article 193 item 1a. of the Polish Aviation Act of 3rd July 2002 (Journal of Law No 130 item. 1112 with changes) a foreign air carrier may perform air transport within the territory the Republic of Poland on condition of possessing the operating licence issued according to the Council Regulation No 2407/92 dated 23rd July 1992. Such operations can be performed only to the extent and on conditions set out in a permission issued by the President of Civil Aviation Office of the Republic of Poland.
The requirements are the same as set forth in points 1,2 and 3 above.
